Effective 1/15/22, your prescription drug benefits will cover up to 8 over-the-counter, at-home COVID-19 tests (or four 2-packs) per person per 30-day period. It is to your advantage to order through the Federal Government Website or use a pharmacy to get at-home COVID-19 tests.
Federal Government
Every home in the U.S. is eligible to order 4 free at-home COVID-19 tests. The tests are completely free. Orders will usually ship in 7-12 days.

Other ways to get free tests through your health insurance
Pharmacies
- Eligible Fund participants with Hospitality Rx drug benefits can get the tests directly from any in-network and out-of-network pharmacy for FREE (with no out-of-pocket costs).
- GO TO THE PHARMACY TO PAY even at a retail store such as Walgreens or CVS; do NOT go to another check-out such as at the cosmetic counter.
- Make sure you have your Hospitality Rx ID card.
- Remember: DO NOT purchase through a grocery store check-out line. If you do, you will have to pay and submit for reimbursement.
Mail Order
- You may also get tests through our mail order vendor, Welldyne.
- Up to 8 FREE tests per month (30-day period) – no shipping charges. Here's how to order:
- When you call, you’ll be asked to agree to the following:
“By submitting this order, I agree that the OTC COVID-19 test is being purchased by the participant, beneficiary, or enrollee for personal use, not for employment purposes, has not been and will not be reimbursed by another source, and is not for resale.”
Reimbursement
- If you did not purchase an at-home test at a retail pharmacy (for example, online at Amazon) on or after 1/15/22, you can submit your receipt for reimbursement. You’ll get up to $12/test or $24/2-pack for eligible purchases based on the cost of the tests.
- Here’s how to get a reimbursement:
- Print out the form.
- Fill out all of the information on the claim form as completely as possible.
- Complete a separate claim form for each family member.
- Include a purchase receipt clearly showing the testing kit charges and date of purchase.

- This is a temporary benefit. FDA-authorized OTC COVID tests that are self-administered and self-read will be covered until the Department of Health and Human Services declares the national emergency related to the coronavirus (COVID-19) has ended.
- COVID-19 testing at your doctor’s office or testing sites or with an order from a healthcare provider’s order are still FREE with your health insurance.
- If you need tests now and cannot find a pharmacy with tests in stock, you may try finding tests online (such as Amazon) and submit it for reimbursement.
- Testing needed for employment purposes is not covered.
- Retiree Plan Unit 173 (UNITE HERE Staff Plan) provides this benefit. The other Retiree Plans do not include this benefit.
If you have Kaiser
- Federal government — Every home in the U.S. is eligible to order 4 free self-tests without shipping fees at covidtests.gov.
- Kaiser Permanente pharmacies — If you’re already planning to visit a Kaiser Permanente location, you may be able to pick up a self-test while they are there, as supply allows.
- Retail pharmacies —pick up self-tests at no cost from participating retail pharmacies. Visit kp.org to see a list of participating retailers. You’ll need to take the test to the pharmacy counter and show your photo ID and Kaiser Permanente ID card. Availability of tests may vary.
- If you pay for an FDA-approved test, submit a claim for reimbursement at kp.org. Select “Coverage & Costs” then select “Submit a medical claim.”
- For the latest information on supply and available brands at Kaiser Permanente locations, visit kp.org/covidtests.
- Getting vaccinated — and getting a booster dose when eligible — is still essential for protecting yourself, your workplace, and your community. Book an appointment for your COVID-19 vaccine or booster at kp.org.

If you have Medicare
- Every home in the U.S. is eligible to order 4 free at-home COVID-19 tests. The tests are completely free. Orders will usually ship in 7-12 days. Order your tests now so you have them when you need them. Shipping begins in late January.

- In addition, the U.S. Department of Health and Human Services (HHS) is providing up to 50 million free, at-home tests to community health centers and Medicare-certified health clinics for distribution at no cost to patients and community members.
If you have state Medicaid and CHIP
- Every home in the U.S. is eligible to order 4 free at-home COVID-19 tests. The tests are completely free. Orders will usually ship in 7-12 days. Order your tests now so you have them when you need them. Shipping begins in late January.
- You should also contact your state Medicaid or CHIP agency for information regarding the specifics of coverage for at-home COVID-19 tests, as coverage rules may vary by state. These programs are required to cover FDA-authorized at-home COVID-19 tests.
