Transplant and CAR-T Network

Starting January 1, 2025, you must use the Fund-designated transplant and CAR-T network to get benefits for transplant and CAR-T therapy services. The Fund’s network includes top-quality, experienced healthcare providers nationwide.

The Plan will only cover transplant and CAR-T therapy (and related services) if you use this special network, which is available through Optum and Cigna LifeSOURCE. This network is different from your regular medical service network.

This page does not apply if you get medical benefits through Kaiser or Tufts/Harvard Pilgrim.

Providers in the Transplant & CAR-T Network

If you need help finding a provider, call HealthCheck360 at (844) 462-7812. (Call HealthCheck360 even if you normally call a different number to find a provider or see a different number on your medical ID card.)

Optum Providers

  • Adult transplant (Adult Transplant Centers of Excellence Network)
  • Pediatric transplant (Pediatric Transplant Centers of Excellence Network)
  • CAR-T (CAR-T/Cellular Immunotherapy Preferred Provider Network)

Cigna Providers

Get prior authorization

Even if you usually call Horizon or eviCore for prior authorizations, transplant and CAR-T authorizations must be made through HealthCheck360.

You or your doctor must call HealthCheck360 before:

  • Getting a transplant evaluation (or any transplant services) – except for cornea transplants.
  • Getting CAR-T services.
