- Proof documents for dependents

What if I don't enroll now?
- If you don’t enroll your dependents now or within 60 days of your first day of coverage (August 1), your dependents will not be covered as of August 1 under UHH and will lose coverage if they’re currently covered by your employer’s plan.
- You can add dependents any time.
- If you add dependents after the first 60 days of your coverage, their coverage begins the first day of the month following the date the Fund receives and approves enrollment and proof documents.
- If you have a qualifying life event; you have 60 days to add dependents. Dependent coverage will begin the first day of the month of the Special Event (see “Special Enrollment Rights” on page 4 for details).
