作為您工會指定的醫療基金，我們致力於確保您的僱主為醫療保健支出所設立的資金池得到保護。為幫助降低成本，我們與一間中立的第三方機構及專業的福利服務公司 SISCO 合作，開展資格審核（審查）工作。

我們希望確保只有符合資格的受養人才能使用***您的***醫療基金款項來支付他們的醫療費用。

##  **需採取行動！** 

**SISCO 已向您寄送一份資料包，其中包含您唯一的審核 ID 代碼。**

您需於 2026 年 1 月 23 日（星期五）之前使用該代碼 [提交受養人的證明文件](https://siscoeligibility.com)。否則，您受養人的醫療保障（保險）可能會在 2026 年 2 月 1 日終止。
